How to take clear photos of your teeth
We will need you to take 8 photos in order for us to get an accurate look at your teeth. By following a few simple steps as demonstrated in the video, we will be able to remotely assess your orthodontic situation. When you have completed taking the needed photos, fill out the form below and submit your photos.
In addition to submitting photos, we will need you to fill out a New Patient Form. Please choose either Child/Teen Form or Adult Form.

It is important that we know a few things about you before we discuss the orthodontic options available to you. Once all of your information and photos are received and reviewed, we will contact you to schedule your virtual consultation using Zoom video conferencing. We reply to all requests within 24 hours, excluding weekends.
